Aftercare: The Underestimated Success Factor
Even the best filler treatment can be compromised by poor aftercare. At the same time, structured aftercare is an important differentiating factor for practices – patients who feel well looked after will return and recommend you to others. This guide provides doctors and clinics with a complete basis for patient education after dermal filler treatments.
Immediate measures directly after treatment (0–6 hours)
- Cooling: Cold compress (not directly on the skin) for 10–15 minutes reduces swelling and bruising
- No massage: Do not massage or press the treated area – filler can shift
- Upright posture: Do not lie down for at least 4 hours (especially after lip treatment)
- No makeup: No makeup on the treated area for at least 6–12 hours
- No alcohol: Alcohol increases propensity to swelling and risk of bleeding
The first 24–48 hours
- No exercise: Increased blood circulation promotes swelling and accelerates HA breakdown
- No sauna, no hot bath: Heat increases swelling and can affect filler distribution
- No solarium, no intense sun: UV radiation accelerates HA breakdown
- No facial massages or treatments: No peeling, no laser, no ultrasound
- Sleep: Prefer sleeping on your back, do not press your face into the pillow
The first 2 weeks
- Sun protection: SPF 50+ daily, even in cloudy weather
- Gentle cleansing: Mild cleansing products, no aggressive peeling
- Hydration: Drinking plenty of water supports the HA effect
- No dental treatments: At least 2 weeks after lip treatment
- No vaccinations: At least 2 weeks' distance (inflammatory reaction possible)
Normal Reactions vs. Warning Signs
| Normal Reaction | When to see a doctor | Emergency – immediately |
|---|---|---|
| Swelling (1–7 days) | Swelling still severe after 7 days | Pale/livid discoloration of the skin |
| Redness (1–3 days) | Increasing redness after day 3 | Severe pain after injection |
| Bruising (1–2 weeks) | Nodules or hardening after 4 weeks | Vision disturbances or eye pain |
| Tenderness (1–3 days) | Fever or general malaise | Capillary refill time >2 seconds |
| Asymmetry (due to edema) | Asymmetry still present after 4 weeks | Facial numbness |
